Telehealth Consent

Telehealth Services

Remmie Care provides access to telehealth-enabled services using remote communication technologies. Telehealth services may include asynchronous (“store-and-forward”) review of medical information, images, videos, questionnaires, messaging, and other electronic communications between patients and licensed healthcare providers.

Medical services are provided by independent licensed healthcare providers and affiliated medical groups using the MD Integrations telehealth platform and provider network.

Remmie Care, a DBA of Remmie Health, Inc., does not practice medicine and does not directly provide medical diagnosis, treatment, prescribing services, or clinical care.

Nature of Telehealth

Telehealth involves the use of electronic communications and technology to enable healthcare providers to review medical information, communicate with patients, and provide healthcare services remotely.

Telehealth services may differ from in-person medical care because providers may not perform a physical examination and may rely on information submitted electronically by the patient.

You understand and acknowledge that:

  • Telehealth may not be appropriate for all medical conditions or concerns.
  • Certain medical conditions may require in-person evaluation, testing, or treatment.
  • Providers may determine that telehealth services are not appropriate for your condition.
  • Providers may recommend that you seek in-person medical care or emergency services.

Asynchronous Care and Response Time

Certain Remmie Care services operate using asynchronous or store-and-forward telehealth workflows. Information submitted through the platform may be reviewed at a later time by a healthcare provider and may not be reviewed immediately.

Response times may vary depending on provider availability, submitted information quality, operating hours, and clinical appropriateness.

Use of Remmie Care does not guarantee:

  • provider review;
  • diagnosis;
  • treatment;
  • prescription issuance;
  • response within a specific timeframe; or
  • availability of services in your state.

Emergency Situations

Remmie Care and the telehealth services available through the platform are not intended for medical emergencies.

Do not rely on messaging, image uploads, email, or telehealth communications for urgent or emergency medical situations.

Device and Image Limitations

Remmie Care may involve the use of connected medical devices, including digital otoscope technology, image capture, audio recordings, questionnaires, and uploaded information.

You acknowledge and understand that:

  • image quality, lighting, positioning, motion, focus, and user operation may affect the quality and accuracy of information submitted;
  • remote evaluation may be limited by incomplete, inaccurate, delayed, or poor-quality information;
  • technical limitations may impact provider assessment;
  • device-generated or software-generated outputs may not identify all conditions, abnormalities, or medical issues.

The Remmie device and related reports are intended to support communication with healthcare providers and are not intended to replace professional medical evaluation, diagnosis, or treatment.

AI-Assisted and Structured Reports

Remmie Care may utilize software tools, automated analysis, structured reports, decision-support tools, or AI-supported technologies to organize or assist review of submitted information.

Any automated analysis, structured report, AI-supported output, or software-generated result is provided for informational and clinical support purposes only and does not constitute a medical diagnosis or independent medical advice.

Clinical decisions, diagnoses, treatment recommendations, and prescription decisions are made solely by the treating licensed healthcare provider.

Prescriptions and Treatment

Healthcare providers using the platform may determine that prescriptions, treatments, referrals, testing, or additional follow-up are appropriate.

Prescription availability may vary based on:

  • state law;
  • provider discretion;
  • medical appropriateness;
  • identity verification;
  • pharmacy limitations; and
  • clinical evaluation requirements.

Providers may decline to prescribe medications or may require in-person evaluation before prescribing certain medications.

Privacy and Electronic Communications

Telehealth services involve the electronic transmission of medical and personal information.

By using Remmie Care, you consent to:

  • electronic communications;
  • electronic transmission of medical information;
  • electronic storage of records;
  • communication by secure messaging, email, SMS, or other electronic means; and
  • sharing of information with healthcare providers, affiliated medical groups, MD Integrations, contractors, and service providers as necessary for treatment, payment, healthcare operations, platform administration, and service delivery.

While commercially reasonable safeguards are used to protect information, no system can guarantee absolute security.

Risks of Telehealth

You understand that telehealth services involve certain risks, including but not limited to:

  • technical failures;
  • interruptions in connectivity;
  • unauthorized access despite security efforts;
  • delayed review;
  • incomplete transmission of information;
  • misunderstandings caused by lack of physical examination;
  • inaccurate image or data capture; and
  • limitations associated with remote care.

You acknowledge that these limitations may affect diagnosis or treatment recommendations.

Consent to Telehealth Services

By using Remmie Care, you voluntarily consent to receive telehealth services through the platform and acknowledge that:

  • you have had the opportunity to review this Consent;
  • you understand the risks and limitations of telehealth;
  • you understand alternatives may include in-person medical care;
  • you may stop using telehealth services at any time; and
  • providers may determine telehealth is not appropriate for your condition.

Changes to This Consent

Remmie Care may update or modify this Telehealth Consent from time to time. Updated versions will be posted through the website or platform with the revised effective date. Continued use of the Services after updates become effective constitutes acknowledgment of the revised Telehealth Consent.
